Note that some forms are not displaying in the Google Docs Gallery site due to a bug. Those that are, can be added to your Google Account from the Google Docs Gallery site. Try any of the following methods. If you have trouble, contact me. 

• view all forms at once at the gallery: display all Sites SOS forms
• input "sitessostemplates"into search box without the quotes
• input form name into search box
